[Scorecard] metadata template

Hi all,

Attached is an example of the kind of metadata record we are looking for.

This is my work-in-progress metadata documentation, I still have some more
to fill in but the format will not change.

It basically reads: data source, data processing, and resultant dataset.
Processing steps can be kept to a minimum as people can reference the report
for more info.
